Abstract

목 적: 덧눈꺼풀로 수술적 교정이 필요한 환아에서 덧눈꺼풀 교정수술을 시행한 뒤 술 전 연령 및 각막난시 정도에 따른 수술 후 환아들의 시력과 각막난시의 변화에 대해 알아보고자 하였다.

대상과 방법: 2009년 1월부터 2011년 12월까지 아래 덧눈꺼풀 교정술을 시행한 환아 총 37명 65안을 대상으로 조절마비굴절검사를 시행하여 최대교정시력과 난시를 측정하여 연령별로 5세 미만, 5세 이상 9세 미만, 9세 이상의 세 군으로 분류하고, 술 전 각막난시에 따라 1D 미만, 1D 이상 2D 미만, 2D 이상 3D 미만, 3D 이상으로 군을 분류하여 술 전 및 술 후 6달, 12달의 최대교정시력과 각막난시의 변화를 비교하였다.

결 과: 환아의 평균 나이는 5.7 ± 2.5세였다. 최대교정시력은 5세 이상 9세 미만 군에서 통계적으로 유의한 호전을 보였으며(p=0.02), 술 전 난시 3D 이상의 군에서 술 후 유의한 호전을 보였다(p=0.039). 술 후 난시의 변화는 5세 이상 9세 미만 군에서 난시의 감소를 보였으며(p=0.004), 1D 이상 2D 미만(p=0.006), 2D 이상 3D 미만(p=0.034), 3D 이상의 군(p=0.015)에서 유의한 난시의 감소를 보였다.

결 론: 아래눈꺼풀 교정술 전 난시의 양이 많을수록 유의한 난시의 감소를 보였다. 따라서 난시가 심한 덧눈꺼풀 환아에서는 보다 적극적인 수술적 처치가 필요하다.

Purpose: To evaluate the changes in visual acuity and astigmatism after epiblepharon surgery regarding preoperative age and astigmatism.

Methods: We retrospectively reviewed the charts of 37 patients with epiblepharon surgery. Preoperative best-corrected visual acuity and degree of astigmatism were obtained. Postoperative data were collected at 6 and 12 months.

Results: The mean patient age was 5.7 ± 2.5 years. In the 5-9 year-old group and ≤3D group, significant improvement of mean BCVA was demonstrated (p = 0.02, p = 0.039, respectively). In the cylinder groups, 1-2 D, 2-3 D, and ≤3D showed decreased mean astigmatism (p = 0.006, p = 0.034, p = 0.015, respectively).

Conclusions: Significant astigmatic reduction was found after surgical correction in epiblepharon patients with higher baseline astigmatism. These results suggest that an epiblepharon surgery should be considered in patients with a high levels of astigmatism.
